Opt-In Form Requirements
SMS opt-in page requirements

In order to qualify for toll-free, our vendor wants to see that you have an SMS opt-in page. It is critical that your opt-in page have these things:
- An optional mobile phone number field
- A stand-alone checkbox where subscribers affirmatively click to opt-in to receive mobile SMS messages. This checkbox cannot be prechecked
- A "call to action" explaining the terms for the SMS program below the checkbox.
- Include the name of the sender, the numerical frequency in which users can expect to receive messages, stop and help instructions, and a "Message and data rates may apply" disclosure.

Note: When describing messaging frequency, a statement like "you can expect to periodically receive text messages" is not enough. You must set a clear expectation with a specific number of texts, for example: "You can expect to receive no more than 1 text per day." or "You will receive a maximum of 10 texts per week."
